Document milestones for agents: CLAUDE.md points at `xo-sdlc --milestones` as the starting query, docs/agents/issue-tracker.md covers the file layout, the `Milestone:` ticket line, and the two properties worth preserving (progress derived / closing deliberate; linkage in the tickets, not the milestone).
